Patient Forms, Map and Directions to Dr. O’Niel’s Office

Your first visit to O'Niel Dental establishes a vital foundation for our relationship with you. During the first visit, we make sure to obtain important background information, like your medical history, and give you time to get to know your doctor. To understand what to expect for your first visit to our practice, please read through this page. You'll find all the practical information you need, such as a map and directions to our office, practice hours, payment policies and more. There's also background information about our committed staff and our first visit procedures. You can even save some time at your first visit by printing out and completing the patient forms in advance of your appointment.

Our mission is to provide you with excellent dental care in a relaxing environment.  We strive to make your dental visit a very pleasant, comforting experience so that you will refer all your friends and family to our office.  We are serious about providing superior dental care at reasonable prices and proud of our dedication to our patients.  Our goal is to help you feel and look your best through excellent dental care.  We are very sensitive to your time constraints and pride ourselves on being on time for every appointment.

We value our patient relationships, making it our priority to deliver the gentle compassionate care that you deserve.   We strive to develop lifelong relationships with our patients by combining the latest dental technology with a professional and compassionate staff.  The result is a beautiful, healthy smile that lasts a lifetime.  We aim to treat each patient as a member of our extended family and we welcome referrals since our existing patients are our best source of new patients.


Patient Forms

Please click on the Patient Registration form below.  You will be able to fill it out online.  Once completed you will be able to send it electronically directly into your record at our office.  If you are not able to do so, please plan to arrive early for your appointment to fill out the paper forms.

Please be sure to bring any insurance eligibility cards and a complete list of medications that you are taking on a regular basis.

What To Expect
Our staff will review your completed registration form and insurance information.  Dr. O’Niel will discuss your medical history and your list of medications.  Be sure to let us know in advance if you need to take antibiotics before your appointment.  We will take any necessary dental x-rays.  You will receive a complete intraoral examination and oral cancer screening.  Finally we will perform a dental prophylaxis (cleaning).  Dr. O’Niel will then share the findings with you and discuss your treatment options.


 


Our Location
We are easy to get to from just about anywhere in the Detroit metropolitan area.  We are less than one mile from I-75.  Our office is located in the Medical Square Complex, which is on Big Beaver Road (16 mile), one block west of Crooks Rd., on the south side of the street. It is a single story complex with red metal roofs.  Coming from the West, go east on Big Beaver Rd, past Somerset Mall. There is a traffic light just before Crooks Rd., immediately in front of our complex. Turn into the driveway just before the light. Go to Building B. Coming from the East, travel westbound on Big Beaver Rd. Take the first turn-around west of Crooks Rd.; turn right into the Medical Square complex. Drive around the back of the complex to get to the west side, we are in building B. Our address is: 1579 W. Big Beaver Rd., Suite B6
